Procedure
A deep plane facelift is a surgical procedure that focuses on enhancing the facial anatomy of a patient by working on a deeper layer of the facial tissue, called the superficial musculoaponeurotic system (SMAS).
This layer consists of dense connective tissue and muscles that provide structural support to the skin and overlying facial tissues.
The procedure is particularly effective in addressing excess skin, volume descent in the midface resulting in flattening of the cheeks, deepened nasolabial folds and jowl formation, as well as improving the contours of the jawline and neck.
The procedure is performed under a general anaesthetic.
Incisions are kept discreet by placing them in natural creases and following the contours of the temple hairline, then extending down in front of the ear and around to the sulcus behind the ear and ending in the hairline.
Step by Step Deep Plane Facelift
- The skin is elevated up to the point where the SMAS layer is to be released and lifted.
- The deep plane SMAS entry point is from the level of the outer corner of the eye extending down across the angle of the jaw and into the neck.
- The SMAS and skin and then lifted together as a single unit, thereby reducing the risk of postoperative haematoma and fluid collection as well as skin contour irregularities.
- The SMAS and skin flap are separated from the deeper tissue and any ligaments holding or restricting the movement of the flap are released thereby allowing the tissue to be lifted. The nerves supplying the muscles of the face are often visualised during this step and care is taken to avoid their damage.
- Once the SMAS flap is released, it is repositioned and secured to achieve a more youthful contour and appearance, correcting the age related changes. If there is excessive fullness and loss of contour of the jawline in the area under the chin, then the SMAS layer may also be tightened in that area as well as removal of any excess tissue. This is normally performed through an incision in the skin crease under the chin.
- The excess skin is removed, and the incisions are closed meticulously, usually with non-absorable sutures before the application of a dressing to support healing and reduce swelling.
Deep Plane Facelift Recovery Timeline
Week 1
Patients generally experience swelling, some bruising and discomfort in the initial recovery period.
As well as simple measures such as keeping the head elevated, pain relief medication, a support garment and cold gel pads are provided to help alleviate the discomfort, reduce swelling and aid the healing process.
Week 2
The skin sutures are usually removed at the end of the first week and scar care instructions are given such as massaging and moisturising and avoiding sun exposure.
It is important to avoid strenuous activity during this early recovery period to help minimise swelling.
Weeks 3-6
Over the following weeks, the swelling and any residual bruising subside, and the final results are realised

How Should I Prepare For Deep Plane Facelift Surgery?
You need to have a consultation with your surgeon and provide details about your medical history, health status, and medications. You may have to adjust your medication schedule or stop taking certain medications entirely.
Smoking should be avoided for six weeks leading up to and after the surgery. It’s also important to arrange for transportation and have a support system during the recovery process.
Can It Be Performed With Other Cosmetic Surgery?
Combining a deep plane facelift surgery with other cosmetic procedures is not uncommon. In fact, many patients opt to have additional procedures performed at the same time to enhance the overall rejuvenation of their appearance. Some common procedures that are performed in combination with a facelift include eyelid surgery and brow lift.
Will I Need A Neck Lift With My Facelift?
A deep plane facelift rejuvenates both the face and neck. Rarely, patients may only desire or benefit from rejuvenation of the face without any treatment to the neck, in which case a mini facelift may be more appropriate.
How Painful Is A Deep Plane Facelift?
The major cause of discomfort post facelift surgery is swelling. This is relieved through the use of cold gel pads, keeping your head more upright when lying and simple oral pain relieving medication which is provided on discharge. These are usually only required for up to the first week after surgery.
How Does A Deep Plane Facelift Differ From Other Facelift Techniques?
Although most modern facelift techniques manipulate the superficial musculoaponeurotic (SMAS) layer, in a deep plane facelift the skin and the SMAS are elevated together as a single unit rather than separately as is the case in traditional SMAS techniques. This reduces the amount of ‘open’ space where fluid may collect. It also reduces the risk of contour irregularities in the skin.
Another key step in a deep plane facelift is the release of all the retaining ligaments holding the SMAS which is not always performed in traditional SMAS techniques. This allows the SMAS to be repositioned more freely with less tension, helping ensure a natural result and avoiding a wind swept appearance.
What Procedures Can Be Combined With A Deep Plane Facelift ?
To achieve optimal facial rejuvenation, additional procedures are often performed in conjunction with a deep plane facelift. This includes brow elevation, eyelid rejuvenation, facial fat grafting, more extensive deep plane neck contouring as well as a nose reshaping (rhinoplasty).
Specific skin treatment may also be performed such as laser resurfacing, chemical peels or microneedling with radiofrequency tightening.
Other regions of the body may also be rejuvenated simultaneously such as breast enhancement and body contouring.
The most important factor to maintain and should always be the paramount concern, is patient safety.
What Scarring Will I Have After This Procedure?
Facelift scars start in the temple, pass along the front border of the ear, before passing around the ear lobe on to the back of the ear and onto the hairline. Additional neck contouring is performed through an incision under the chin. To ensure the scars are kept discreet, the incisions are positioned along the natural creases and contours and are closed meticulously.
How Long Will The Results Last?
On average the results of a deep plane facelift can last between 7-10 years. This can vary and depends on a multitude of factors such as changes in weight, smoking and genetics, as well as the fact that the rate the natural ageing process does not alter with surgery. Good skin care and a healthy lifestyle help maintain and prolong the results of the procedure.
How Soon Could I Return To Work After A Deep Plane Facelift?
Patients can expect to return to work 10-14 days after surgery. This can vary depending on the degree of swelling and bruising experienced, as well as the nature of the work being performed.
